Eutrapio Sarmiento (Jun) Guevara completed his undergraduate degree of B.S. Pre-Med at the University of Santo Tomas in 1968 before earning his Medical Degree in 1973 also at the University of Santo Tomas, Espana, Manila. He received his ENT residency training program at the UP-PGH and his final year as Chief Resident of the Department of ENT (1978).
Dr. Guevara has completed several years (5-6 yrs) of post-training fellowship in Europe and the USA with a grant from UP-Philippine Government thru NEDA in 1978. Before proceeding to Europe, he undertook a three-month visiting fellowship training in Head and Neck Plastic-Reconstructive Surgery in Roswell Park Cancer Memorial Center at the State University of Buffalo, New York, USA under Dr. Vahram Bakamjian and Donald Shedd. Seeking further training in Europe, he started his integrated fellowship program in Head and Neck Plastic-Reconstructive Surgery under the British Council sponsorship in UK (London-St. George’s Hospital, Hospital for Sick Children, Royal Marsden Hospital and Queen Mary’s Hospital as Rotating Senior Registrar); Edinburgh with Dr. Arnold Maran; Glasgow-Canniesburn Hospital with Mr. Ian McGregor and John Clarke Mustarde; University Hospital Liverpool with Prof. Philip Stell; and at the Oxford University John Radcliffe Infirmary Hospital. In France under Alliance Francaise sponsorship, he trained with Dr. Paul Tessier at Hopital Foch, Suresness, Paris for his CranioMaxillofacial Surgery and with Dr. Daniel Marchac for Craniofacial and Plastic-Reconstructive Surgery at the Hopital Necker Enfants Malades in Paris. In Switzerland(Hoffman La Roche) he trained with Prof. Hugo Obwegeser for Maxillofacial Surgery and Dr. Leo Clodius for Plastic Surgery in Zurich, and with Dr. Rudolphe Meyer for Cosmetic Surgery of the Face in Lausanne, Switzerland. In Germany, he trained with Prof. Hans George Luhr at the University Hospital Gottingen; and with Prof. Widmaier, Dr. Heinz Riechert, and Dr. Wolfgang Gubisch for Plastic and Maxillofacial Surgery at the Marriene Hospital, Stuttgart, Germany. During this fellowship, he augmented surgical training in Cleft lip/palate, Orthognathic and Aesthetic Maxillofacial Surgery, Craniosynostosis, Reconstructive microsurgery, Head and Neck cancer reconstructive surgery, Facial reanimation, Craniofacial trauma, and Facial cosmetic and corrective secondary facial surgery-Cleft Lip/Noses and Septorhinoplasty.
Upon his return in the Philippines in 1984, he was given an academic and clinical appointment at the UP-PGH as Professor 1 in the Department of Otolaryngology. At that time in 1984, he recommended to the PBO to establish a section service of Facial Plastic-Reconstructive, Cosmetic and Maxillofacial Restorative Surgery in all departments of ENT-HNS training program. He started then to run a series of workshops, seminars, surgical demo and postgraduate courses in Head and Neck Surgery, Plastic-Reconstructive, Cosmetic and Maxillofacial Surgery and Trauma for ENT specialty. Dr. E. Guevara Jr. also initiated the Basic courses in Facial Plastic-Cosmetic-Reconstructive and Maxillofacial Surgery and Trauma which made ENT specialty at par with other plastic surgery specialties in the Philippines. He was the first to introduced Titanium plating in Maxillofacial Surgery and Trauma open reduction repair in 1989. He was inducted as a Fellow of the PSO-HNS in 1984 and became very active in the ENT-HNS Society activities. He was always invited as organizer and speaker in PSO- HNS Annual Conventions and Post-graduate courses and symposiums and was very active in international conventions of ENT, Facial Cosmetic Surgery and Maxillofacial Surgery Societies in Asia as a speaker.
His private practice in ENT-HNS, Facial Plastic-Reconstructive Surgery started at St. Luke’s Medical Center, Quezon City in 1986 and was appointed as Chairman of the Dept. of ENT-HNS in 1989 until 2005. The Dept. of ENT-HNS of St. Luke’s Medical Center, Quezon City was the first private hospital institution fully accredited training program in ENT (1992). Dr. E.S. Guevara Jr. also established the first fully equipped Voice-Swallowing and Sinus Center at St. Luke’s Medical Center in the Philippines (2002) after a brief tutorial with Dr. Robert Sataloff, Dr. Jonathan Aviv and Dr. James Kaufmann from USA. He introduced Microlaryngel Laser Surgery with some of his younger staff trained in Voice Surgery and Care of the Professional Voice Users, Radiofrequency surgery using the Coblator in Endoscopic Sinus and Oropharyngeal surgery (1999) and Image-Guided Surgery (Brain Lab) for Sinus Surgery (2003). He was also very instrumental in establishing Sleep Center at St. Luke’s Medical Center, Quezon City in 2003 which became now an active subspecialty in ENT. He is also active in doing Sleep Apnea, Multi-level Obstructive Sleep Apnea surgery, TurbinoSeptoplasty, Maxillo-Mandibular Advancement, Maxillary Expansion (DOME) at present the Endoscopic Assisted Surgical Expansion (EASE).
Dr. Guevara Jr. was past president of PSO-HNS in 2007. When he was President of the PSO-HNS he made all study groups of PSO-HNS as an academy and became a founding member of PAFPRS, AHNOP, PAR, ENT-SLEEP ACADEMY and PALP. He is now retired at UP-PGH (Prof. IV) but still remains an active consultant at the Department of ENT-HNS, St. Luke’s Medical Center, Quezon City. His works in Head and Neck Surgery-Facial Cosmetic, Plastic-Reconstructive Surgery, Craniofacial-Aesthetic Maxillofacial Surgery are now beginning to focus on 3D Virtual Surgical Planning and Sleep Apnea surgery. This is exemplified by his pioneering work again first in the country regarding Adult Temporomandibular Joints (TMJ) End-Stage Pathology due to neglected trauma and other TMJ diseases and disorders (2014). The patient is custom-fitted (CAD/CAM-CBCT/Ct-Scan) with total TMJ prosthetic implants mimicking the normal anatomy of the complex TMJ and mandibular function to provide long-term effective outcomes when other surgical techniques failed (2017). This is all done in 3D Printing Virtual Surgical Planning which is the most recent advanced technology in CranioMaxillofacial Surgery which he is now applying for his Orthognathic surgery, Craniofacial deformities, Head and Neck Cancer Reconstructive Surgery and Complex Maxillofacial Trauma.
In Facial Cosmetic Surgery, Dr. Guevara Jr. pioneered the Open Structured Rhinoplasty way back in 1988 here in the Philippines wherein most of the Rhinoplastic surgeons were using the Close Technique with bilateral rim incisions in their Augmentation Rhinoplasty. Open structured Rhinoplasty is now being applied and becoming popular to most ENT Facial Rhinoplastic surgeons. He also started to use SAM-GoreTex (USA, e-PTFE)for augmentation rhinoplasty way back in 1997 even before the Koreans started popularizing it in 2005. He also popularized the Septorhinoplasty of the Crooked Nose because of his exposure to Professors Wolfgang Gubisch and Rettinger both from Germany in 1982. His training in Craniofacial Surgery and Cleft Lip and Palate Surgical repair also made him an expert in Revision Cleft Lip/Noses and Orthognathic Surgery as well as in Aesthetic Craniomaxillofacial Surgery to achieve Facial Balance, Symmetry and Form. He started the use of Polycaprolactone (PCL) by Osteopore, Singapore for Deviated Nasal Dorsum and Tip Plasty in 2016. He is at the moment undertaking a clinical trial on the use of Polycaprolactone (PCL) plus Tricalcium phosphate, Hydroxy-apatite, Platelet-Rich Plasma or BMX (PCL-TCP+PRP) for bone regeneration effect in the reconstruction of facial bone atrophy or defects such as the mandible in the 3D Printed process. This study is in collaboration with OSTEOPORE, Singapore but was temporarily halted because of the pandemic years 2020 and 2021.
